Instructions
- Supervisors – Complete form, sign and select the “Save & Send for Review Option”.
- Enter the email address of the employee or copy and paste the link and share it with them.
- Employee – Please review, sign and finalize form by clicking the “Finalize and Print” button.
- After submitting, you can either print or save the form as a PDF for your records.
- Please send either a print copy or a PDF file to your supervisor so they can keep it for their records.
